Quality Improvement Storyboards:
Six Domains of Health Care Quality
In order to create measurable goals for the organization, KPHC has adopted the six domains of health care quality from the Institute of Medicine (IOM). These six domains are as follows:
Safety: Avoiding harm to patients from the care that is intended to help them.
Effective: Providing services based on scientific knowledge to all who could benefit and refraining from providing services to those not likely to benefit (avoiding underuse and misuse, respectively).
Patient-centered: Providing care that is respectful of and responsive to individual patient preferences, needs, and values and ensuring that patient values guide all clinical decisions.
Timely: Reducing waits and sometimes harmful delays for both those who receive and those who give care.
Efficient: Avoiding waste, including waste of equipment, supplies, ideas and energy.
Equitable: Providing care that does not vary in quality because of personal characteristics such as gender, ethnicity, geographic location and socioeconomic status.
Kewa Pueblo Health Corporation continues to strive for high standards when it comes to patient care and its medical services. KPHC’s mission is to ensure health and wellness through excellence in health care and in order to accomplish this, delivering high quality patient care is central to the mission.
We never stop trying to do a better job of caring for those we serve.
KPHC strives to improve the quality and safety of our care by implementing new metrics as well as collaborating with departments on new process improvement projects.
We want you to feel confident that you will receive the finest possible care when you come to us. Anything less would fall short of what the community expects from us—and what we expect from ourselves.
The Quality Improvement Department collaborates with the Medical Staff providers, nursing departments, and all other KPHC departments to develop integrated solutions that augment best practices, which result in high quality care and service. Process improvement is ongoing and never ending as we strive to utilize the latest quality and evidence-based strategies to provide the highest quality of care to our patients.
As we move forward with improving processes, you might see new and different changes happening within the facility. These are to help you, the patients, have a better experience at KPHC and improve health outcomes.
